The Company

Storm3’s client is a growth-stage healthcare technology company focused on improving diagnostic accuracy and patient outcomes through a combination of clinical expertise, AI, and large-scale healthcare data. Following a recent strategic combination with a complementary healthcare services business, they’re building a more connected experience across the radiology care journey and already support millions of covered lives through relationships with major employers, health plans, and value-based care organizations.

The Role

As VP of Population Health, you’ll lead and scale a proven population health program while owning relationships with health plans, provider groups, and primary care partners. This is a role for someone who understands Medicare Advantage, can navigate payer-provider dynamics, and is comfortable being accountable for both operational performance and customer outcomes.
You’ll work closely with commercial, product, analytics, and executive leadership teams to grow the program, drive adoption, and deliver measurable clinical and financial results. The environment is fast-paced and hands-on, so success here comes from being able to build trust with partners, make decisions, and execute without relying on heavy processes or layers of management.

Qualifications:

• Deep Medicare Advantage expertise
• Strong understanding of value-based care and risk-sharing models
• Experience working directly with payers, providers, or both on high-priority initiatives
• Background in customer success, account management, population health operations, or a related function

Responsibilities:

• Own performance of the Population Health program from data intake through gap closure and outcome validation
• Serve as executive relationship owner for health plan and provider partners
• Align commercial strategy and operational delivery to support successful customer outcomes
• Drive adoption across ACOs, PCP groups, and radiology partners
• Shape and execute the broader Population Health strategy alongside executive leadership
• Lead implementation and deployment of new population health partnerships
• Partner closely with product, technology, AI, analytics, and operations teams
• Establish and track KPIs measuring program success and business impact
• Build trusted executive-level relationships through proactive partner engagement
